About the Role

The Royal Devon and Exeter NHS Foundation Trust is seeking a highly skilled Clinical Practitioner to join our Stroke Rehabilitation Unit as part of the Stroke Department within the Medicine Care Group. This is a fixed-term appointment for 2 years, offering an exciting opportunity to enhance inpatient care and discharge from hospital of patients undergoing rehabilitation following a stroke.

Key Responsibilities
  • Develop and lead the rehabilitation beds on the stroke rehabilitation unit, including admission, discharge, and patient care.
  • Work across the entire stroke pathway to facilitate the development of a coordinated service for patients with stroke and those needing stroke-rehabilitation within the area.
  • Maintain and develop relationships with external partners, including specialist rehabilitation units, to optimize transitions of care.
  • Be accountable for the delivery of stroke rehabilitation.
Requirements

Suitably qualified applicants from nursing and AHP backgrounds who are passionate about the holistic, multidisciplinary care and rehabilitation of patients who have experienced a stroke are encouraged to apply. Prescribing/deprescribing is an essential part of the role, making it less suitable for individuals with an Occupational Therapy background.

The successful candidate will work a variable schedule, with a focus on delivering high-quality patient care and contributing to the development of our stroke rehabilitation services.
