About The Role

Transforming Urgent Care Delivery

Practice Plus Group is seeking an ambitious and committed Advanced Clinical Practitioner to join our multidisciplinary team in Devon. As a key member of our Out Of Hours and Clinical Assessment service, you will play a vital role in improving the way we deliver urgent care to our patients.

Our Integrated Urgent Care model brings together NHS 111, Clinical Assessment Service, and other urgent care services to provide patients with seamless access to the right care, when they need it. As an Advanced Clinical Practitioner, you will be empowered to work autonomously, assessing patients, planning their care, administering treatment, and discharging or referring them to other healthcare professionals via primary care centers and home visits.

You will be working closely with a friendly and close-knit team of GPs, registered nurses, UCPs, mental health nurses, and dental nurses. Our electronic prescribing system will enable you to streamline patient care and make a tangible impact on the healthcare landscape.

Key Responsibilities
  • Assess patients and develop personalized care plans
  • Administer treatment and prescribe medication as needed
  • Discharge or refer patients to other healthcare professionals
  • Collaborate with the multidisciplinary team to ensure seamless patient care
