About the Role

The Betsi Cadwaladr University Health Board is seeking a highly motivated and experienced Deputy Home First Coordinator to join our multi-disciplinary Home First Team in Wrexham. As a key member of the team, you will play a vital role in promoting the Home First ethos and providing high-quality care to patients in their own homes.

Key Responsibilities

  • Deputise for the Home First Coordinator and provide leadership and guidance to the team.
  • Screen patients and identify care availability to facilitate safe and timely discharge from hospital.
  • Work with the Discharge to Assess model of care to provide ongoing support to patients with complex needs.
  • Act as a link between acute and community services and work with the Community Resource Team to promote seamless care transitions.
  • Contribute to the development of the Home First service and identify opportunities for improvement.

Requirements

  • NMC Registered first-level nurse with significant community experience.
  • Evidence of ongoing education and professional development to Diploma Level or equivalent level of experience.
  • Mentorship and Assessment training.
  • Excellent analytical, organisational, and communication skills.
  • Ability to work collaboratively with members of the primary health care team and other agencies.

Desirable Qualifications and Experience

  • Management qualification (ILM, RCN Leadership).
  • Independent Prescribing.
  • District Nurse Specialist qualification or equivalent community nursing/development qualification.
  • Link Nurse role experience.
