About the Role

We are seeking a dedicated and compassionate Community Reablement Assistant to join our team at Shared Services Partnership. As a Community Reablement Assistant, you will play a vital role in supporting individuals who have experienced a major setback in life to regain their independence and confidence.

Key Responsibilities
  • Provide person-centred support to individuals to help them regain their ability to perform daily tasks
  • Assist with washing, dressing, and managing continence needs
  • Offer practical hands-on support and verbal guidance and encouragement
  • Work collaboratively with the individual, their family, and other healthcare professionals to develop a personalized support plan
  • Contribute to a safe and supportive environment that promotes the individual's wellbeing and independence
Requirements
  • Enthusiastic and motivated individual with a caring attitude
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to maintain confidentiality and handle sensitive information
  • Willingness to undergo training and development opportunities
