About the Role

We are seeking a skilled and compassionate Community Reablement Assistant to join our team at Shared Services Partnership. As a Community Reablement Assistant, you will play a vital role in supporting individuals who have experienced a major setback in life to regain their independence and confidence.

Key Responsibilities
  • Provide person-centred support to individuals in their own homes, promoting their independence and well-being.
  • Assist with daily living tasks, such as washing, dressing, and managing continence needs.
  • Offer practical hands-on support and verbal guidance to help individuals regain their confidence and ability to perform daily tasks.
  • Work collaboratively with other healthcare professionals to ensure a comprehensive and coordinated approach to care.
  • Maintain accurate records and reports to ensure effective communication and coordination of care.
Requirements

To be successful in this role, you will need:

  • A caring and compassionate approach, with a focus on person-centred care.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and their families.
