We are seeking a dedicated and compassionate Community Reablement Assistant to join our team at Shared Services Partnership. As a Community Reablement Assistant, you will play a vital role in supporting individuals to regain their independence and confidence after a hospital admission.
Key Responsibilities:
- Support individuals with daily living tasks, such as washing, dressing, and managing continence needs.
- Provide practical hands-on support and verbal guidance to help individuals regain their independence.
- Work collaboratively with the multidisciplinary team to develop and implement personalized care plans.
- Conduct regular assessments to identify individuals' needs and develop strategies to support their recovery.
- Maintain accurate records and reports to ensure effective communication with the team and stakeholders.
Requirements:
- Enthusiastic and motivated individual with a caring attitude.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Valid driver's license and access to a reliable vehicle.

Working Patterns:
Our Service runs 07:00-22:00, 7 days a week, 365 days of the year. You will be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid.
Travel Requirements:
You will be required to travel to various locations within Hampshire and it is essential that you have a current drivers' license and access to a reliable vehicle on a regular basis.
