Job Summary:

We are seeking a dedicated and compassionate Community Reablement Assistant to join our team. As a Community Reablement Assistant, you will play a vital role in supporting individuals to regain their independence and confidence after a hospital admission or other life-changing event.

About the Role:

Our Community Reablement Assistants work with people of all ages and backgrounds, providing practical support with daily tasks, managing continence needs, and offering verbal guidance and encouragement. You will work in a person-centred approach, focusing on an individual's strengths to maximise their wellbeing.

Key Responsibilities:
  • Provide practical support with daily tasks, such as washing, dressing, and managing continence needs
  • Offer verbal guidance and encouragement to help individuals regain their independence and confidence
  • Work in a person-centred approach, focusing on an individual's strengths to maximise their wellbeing
  • Travel to various locations within Hampshire, using a reliable vehicle
Requirements:

To be successful in this role, you will need to be enthusiastic, motivated, and patient with a caring attitude. You will also need to have a current driver's licence and access to a reliable vehicle on a regular basis.
