Community Support Facilitator

About the Role:
As a Community Support Facilitator at Hampshire County Council, you will play a vital role in assisting individuals who have experienced significant life challenges. Your primary responsibility will be to help them regain their independence and confidence.
Many of our service users may be recovering from hospital stays, surgeries, or falls, and our dedicated Reablement Service is tailored to support them in reclaiming their daily living skills in a safe and effective manner.
In this position, you will work closely with clients to aid them in activities such as personal hygiene, dressing, and managing their daily needs. You will provide both practical assistance and verbal encouragement, with the ultimate goal of fostering their ability to perform these tasks independently.

The Reablement Team offers a comprehensive induction program along with ongoing training and development opportunities. There are clear pathways for career advancement, with many former Community Support Facilitators progressing to roles such as Occupational Therapists or Registered Managers.
Requirements:
Travel to various locations within Hampshire is necessary, so a valid driver's license and access to a reliable vehicle are essential. The Reablement Service operates seven days a week, and you will be expected to work evenings, weekends, and public holidays as part of your regular schedule.
