About the Role:

We are seeking enthusiastic and motivated individuals to join our Reablement Team as Community Reablement Assistants. As a Community Reablement Assistant, you will play a vital role in supporting individuals who have experienced a major setback in life, helping them to regain their independence and confidence.

Key Responsibilities:

  • Work with service users to regain independence with daily tasks, providing practical hands-on support and verbal guidance.
  • Assist service users with washing, dressing, and managing continence needs.
  • Develop and maintain positive relationships with service users, their families, and other healthcare professionals.
  • Contribute to the development and implementation of individualized care plans.
  • Participate in ongoing training and development to enhance skills and knowledge.

Requirements:

  • Current drivers' license and access to a reliable vehicle.
  • Ability to work a shift pattern, including evenings and weekends.
  • Excellent communication and interpersonal skills.
  • Ability to work as part of a team and maintain confidentiality.
