Job Summary:

We are seeking a skilled Care and Support Professional to join our team at Barchester Healthcare. As a key member of the care team, you will be responsible for delivering high-quality care and support to our residents, ensuring their physical, psychological, and social needs are met.

About the Role:

This is an excellent opportunity for an experienced Registered Nurse looking to take on new challenges and responsibilities. You will work collaboratively with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.

Main Responsibilities:

Deliver high-quality care and support to residents, promoting independence and dignity.Assess and meet the physical, psychological, and social needs of residents through comprehensive care planning.Work collaboratively with the care team to ensure residents receive comprehensive care, responding promptly to their changing needs.Develop and implement individualized care plans that address the specific needs and goals of each resident.Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
