Community Health Advocate

About the Role

We are seeking a highly skilled and compassionate Community Health Advocate to join our team at Saludos Hispanos. As a Community Health Advocate, you will be responsible for providing case management services to diverse communities, responding to client needs, advocating for access to community services, and promoting self-sufficiency in meeting short and long term needs and goals.

Key Responsibilities
  • Client Case Management: Provide client-driven flexible support to program participants, including information, translation, advice, referrals, and advocacy services in the client's native language.
  • Case Note Maintenance: Maintain thorough case notes regarding client interactions and issues.
  • Agency Advocacy: Advocate for clients while speaking with other agencies, organizations, caseworkers.
  • Transportation Services: Provide transportation to clients for appointments.
  • Progress Reporting: Report on progress of client issues to Supervisor via informal briefings, case conferences, and team meetings as required.
Requirements
  • Culture Knowledge: Knowledge of culture and experience in providing trauma-informed care or willingness to learn.
  • Language Proficiency: Spanish and English language proficiency required.
  • Caseworker Experience: Prior experience as a caseworker for public or nonprofit employer.
  • Social Program Implementation: Prior experience implementing social programs.
Preferred Qualifications
  • Training and Development: Attend training or conferences to remain current with trends and best practices in service delivery.
  • Research Skills: Ability to research and address the social determinants of health and how financial stressors affect individuals.

Estimated Salary: $55,000 - $65,000 per year
