Experienced Community Health Practitioner

NHS Scotland, a leading healthcare provider, is seeking an experienced and skilled professional to join our community nursing team in Dundee. The ideal candidate will be a registered general nurse with advanced clinical assessment skills and a commitment to delivering high-quality care.
The successful applicant will work collaboratively with colleagues across the locality to achieve the objectives of the Dundee Health & Social Care Partnership Strategic Plan. This role requires excellent leadership skills, including managing workload and a skill-mix team of staff, as well as strong interpersonal and communication skills.

About the Role
- To deliver high-quality, effective care to patients in Dundee, working closely with colleagues and other healthcare professionals.
- To provide leadership and guidance to the community nursing team, ensuring that all staff members have the necessary support and resources to deliver exceptional patient care.
- To contribute to the development and implementation of new models and pathways of care within the service, working collaboratively with multidisciplinary teams.
- To maintain accurate records and ensure compliance with relevant policies and procedures.
Requirements
- A Registered General Nurse with experience in community nursing.
- Advanced clinical assessment skills and qualifications in Non-medical prescribing and PG Cert Integrated Community Nursing.
- Excellent leadership skills, including the ability to manage workload and lead a team.
- Strong interpersonal and communication skills, with the ability to build relationships with patients, families, and healthcare professionals.
Salary and Benefits
The salary for this role is £43,141 per annum, plus benefits including pension scheme, annual leave, and access to ongoing professional development opportunities.
