About the Role

We are seeking a skilled and compassionate Community Reablement Assistant to join our team. As a Community Reablement Assistant, you will play a vital role in supporting individuals to regain their independence and confidence after a hospital admission.

Key Responsibilities
  • Provide practical support with daily tasks, such as washing, dressing, and managing continence needs.
  • Offer verbal guidance and encouragement to help individuals regain their independence.
  • Work collaboratively with the community team to develop personalized support plans.
  • Travel to various locations within Hampshire to provide support to individuals in their own homes.
Requirements

To be successful in this role, you will need to have a caring attitude, be patient, and have a person-centred approach. You will also need to have a current driver's license and access to a reliable vehicle.

Working Patterns

Our Service runs 07:00-22:00, 7 days a week, 365 days of the year. You will be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid.

Travel Requirements

You will be required to travel to various locations within Hampshire and it is essential that you have a current driver's license and access to a reliable vehicle.
