About the Role

We are seeking a highly skilled and experienced Registered Senior Nurse to join our Virtual Ward team as a Clinical Lead. This is an exciting opportunity to be part of a pioneering initiative that is transforming the way we deliver care to patients.

Key Responsibilities
  • Lead the development and implementation of virtual ward pathways, working closely with multidisciplinary teams to ensure seamless care delivery.
  • Assess and plan individualized patient care, taking into account the patient's clinical needs and preferences.
  • Facilitate the use of digital health solutions to support patients with long-term conditions, working closely with community services and other stakeholders.
  • Collaborate with the Virtual Ward Manager and other key stakeholders to develop and implement strategies to improve patient outcomes and reduce hospital admissions.
  • Provide clinical leadership and guidance to nursing staff, ensuring that they have the necessary skills and knowledge to deliver high-quality care.
  • Contribute to the development of policies and procedures related to virtual ward care, ensuring that they are evidence-based and aligned with national guidelines.
Requirements
  • NMC Registered Nurse (Adult) or HCPC Registered Paramedic.
  • Recognized teaching or mentorship qualification at degree level or equivalent.
  • Management and leadership module or study days/proven evidence of.
  • Proven knowledge of Government policies relating to Virtual Ward and Clinical Governance.
