Senior Virtual Ward Nurse

We are seeking an experienced and skilled Senior Virtual Ward Nurse to join our team at University Hospitals Plymouth NHS Trust. As a key member of our Virtual Ward team, you will play a vital role in delivering high-quality patient care and contributing to the development of our virtual ward services.
Key Responsibilities- Assess and plan individualized patient care, taking into account the patient's clinical needs and preferences.
- Develop and implement effective care plans, working collaboratively with multidisciplinary teams to ensure seamless care transitions.
- Provide expert clinical guidance and support to patients, carers, and healthcare professionals, promoting evidence-based practice and best outcomes.
- Contribute to the development and implementation of virtual ward pathways, working closely with stakeholders to ensure effective service delivery.
- Lead and mentor junior staff, promoting a culture of excellence and continuous learning.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.
- Registered Nurse (Adult) or HCPC Registered Paramedic with a proven track record of delivering high-quality patient care.
- Recognized teaching or mentorship qualification at degree level or equivalent.
- Management and leadership module or study days/proven evidence of.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
- Ability to work autonomously and as part of a team, with a strong focus on patient-centered care.
