About the Role

We are seeking an experienced and skilled Senior Virtual Ward Nurse to join our team at University Hospitals Plymouth NHS Trust. As a key member of our Virtual Ward team, you will play a vital role in delivering high-quality patient care and contributing to the development of our virtual ward services.

Key Responsibilities
  • Assessment and Planning: Conduct comprehensive assessments of patients to determine their clinical needs and develop individualized care plans.
  • Clinical Leadership: Provide clinical leadership and expertise to support the development of virtual clinical remote monitoring pathways and work closely with community teams and hospitals to promote the benefits of virtual wards.
  • Collaboration and Communication: Work effectively with multidisciplinary teams, including primary care, community services, and acute hospital services, to ensure seamless care delivery and coordination.
  • Education and Training: Provide education and training to staff on the use of digital health solutions and promote effective use of community services.
  • Quality Improvement: Contribute to quality improvement initiatives and ensure that care delivery meets local and national standards.
Requirements
  • Qualifications: NMC Registered Nurse (Adult) or HCPC Registered Paramedic.
  • Experience: Proven experience in a clinical leadership role, with a strong understanding of government policies relating to virtual wards and clinical governance.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
