About the Role

We are seeking an experienced Clinical Pharmacist to join our team at Livewell Southwest as an Advanced Clinical Pharmacist for Virtual Wards. This is an exciting opportunity to work in a dynamic and innovative healthcare environment, providing high-quality, patient-centred care to individuals with frailty in our communities.

Key Responsibilities
  • Lead and deliver a high-quality, clinical pharmacy service to Virtual Ward areas, working in partnership with our System partners to maximise seamless transfers of care and best outcomes for patients.
  • Coordinate clinical pharmacy services to the Virtual Wards, utilising a holistic approach to support medicines optimisation in various settings.
  • Develop and lead clinical audits and quality improvement initiatives within Virtual Wards, with opportunities to be involved in research and further develop your career.
  • Support and develop the Virtual Ward pharmacy team and other healthcare professionals within Virtual Wards, ensuring high clinical pharmacy standards.
Requirements
  • Masters Degree or equivalent in Pharmacy.
  • Registered Practising Member of the General Pharmaceutical Council.
  • Specialist knowledge acquired by postgraduate diploma or equivalent evidence of specialist knowledge, competence, and achievements in practice in pharmacy.
  • Evidence of Continuing Professional Development and revalidation.
Desirable Qualifications and Experience
  • Postgraduate qualification in clinical pharmacy.
  • Member of specialist interest groups.
  • Working towards membership of the RPS Faculty.
  • Independent prescriber or willingness to work towards.
Skills and Abilities
  • Excellent communication and interpersonal skills, including verbal, written, and formal presentation skills for a wide variety of audiences.
  • Computer literate, including Microsoft Word, Excel, PowerPoint, Email, and Internet.
  • Excellent critical appraisal and analytical skills, with a sound understanding of evidence-based medicine.
  • Good listening skills and excellent networking/negotiating skills.
  • Leadership and team working skills.
  • Ability to act as an excellent role model and mentor.
  • Self-motivated and able to motivate others with enthusiasm.
  • Able to reflect on and appraise own performance and that of others.
  • Ability to plan and prioritise own work and that of others with good organisational and time-management skills.
  • Able to work under pressure and to tight deadlines.
  • Ability to travel across the LSW area.
