Dietitian - Frailty Virtual Ward Specialist

We are seeking a highly skilled and experienced Clinical Dietitian to join our team at Livewell Southwest. As a key member of our multidisciplinary team, you will play a critical role in providing specialist nutrition and dietetic services to patients on our Frailty Virtual Wards.
Key Responsibilities- Provide specialist dietetic assessments, diagnoses, treatments, and education to individual patients on the Frailty Virtual Ward.
- Offer advice, signposting, guidance, and information to healthcare professionals, patients, carers, relatives, or other non-professional contacts.
- Be an integral member of the multidisciplinary team, providing education, training, and dissemination of information to all healthcare professionals.
- Work as an autonomous practitioner, holding a clinical caseload and working without direct supervision.
- Continuously develop your professional skills and knowledge in line with HCPC regulations.
- BSc in Human Nutrition and Dietetics or related qualification and 3 or more years of demonstrable relevant experience.
- HCPC Registration.
- Evidence of ongoing professional development.
