Care Manager Job Opportunity:

We are seeking an experienced Care Manager to join our team at Progress Care Solutions. As a key member of our management team, you will be responsible for ensuring the highest standards of care and support for our young people with complex needs.

About the Role:
This is a challenging yet rewarding role that requires a high level of skill and dedication. As a Care Manager, you will work closely with our senior management team to develop and implement care plans that meet the unique needs of each young person. Your excellent communication skills will enable you to build strong relationships with our residents, their families, and other stakeholders.

Key Responsibilities:
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team.
  • Conduct regular risk assessments and reviews to ensure the safety and well-being of our residents.
  • Collaborate with colleagues to provide high-quality care and support that promotes independence and inclusion.

Requirements:
  • Diploma in Health and Social Care (Children and Young People) or equivalent.
  • Minimum 1 year's experience as a team leader in a residential setting.
  • Strong communication and IT skills.
  • Ability to communicate on various levels.
  • Working knowledge of care plans.

Estimated Salary Range: We estimate the salary range for this role to be between £28,000 - £33,000 per annum, depending on experience.
