Job Overview

The Family Health Practitioner will work as part of a skilled team delivering the Healthy Child Programme to school-aged children in the Wakefield District. The post holder will work to address health inequalities and public health priorities within the area.

Main Duties

Supporting the 5-19 Family Health Team in delivering targeted interventions using a tiered approach. Working with other agencies to ensure a coordinated approach to addressing health inequalities. Contribute to the development, delivery, and reporting of health-promoting activities within various settings.

Deliver evidence-based early intervention programmes to support children, young people, and families. Maintain contemporaneous and accurate records as required by HDFT and in line with professional standards.
