About the Role

We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at Riverside Health Partnership. As a key member of our Primary Care Network, you will play a vital role in delivering high-quality patient care and contributing to the development of innovative healthcare services.

Key Responsibilities

  • Undertake clinical nursing practice at an advanced level, using expert knowledge and clinical skills to deliver holistic care to patients accessing primary healthcare services.
  • Assess, diagnose, and treat patients with a range of acute, non-acute, and chronic medical conditions.
  • Directly admit patients to secondary care with acute medical needs and refer patients to other medical specialties as appropriate.
  • Work collaboratively with multidisciplinary teams, secondary care, and other statutory and voluntary organizations to ensure seamless patient care.
  • Interpret diagnostic tests and routine clinical procedures, making critical clinical judgments to satisfy the expectations and demands of the role.

Person Specification

  • Essential:
    • Understanding of computer-based medical records management systems (SystmOne/EMIS).
    • Significant post-registration experience.
    • Recent primary and community nursing experience.
    • Nurse-led management of minor illness, minor ailments, and injuries.
  • Desirable:
    • Worked in a General Practice environment.
    • Working with community development initiatives.
    • Health-needs assessment.
    • Compiling protocols and clinical guidelines.
    • Audit.
    • Research.
