About the Role
We are seeking a skilled and experienced practitioner to join our team as an Integrated Offender Health Practitioner. The successful candidate will work within our Health and Justice service, supporting individuals with complex needs and vulnerabilities in the criminal justice system.
Main Responsibilities
The post holder will be responsible for conducting specialist assessments and interventions for individuals with diverse needs, including those in custody, court, community, and prisons. They will assess, access, and implement a wide range of care services, formulating and agreeing care and support plans with adults and carers.
Person Specification
The ideal candidate will possess a relevant degree or diploma level qualification in Nursing, Occupational Therapy, or Social Work, and have experience working in a similar role. They will be able to work independently, assess complex needs, and provide clinical supervision to other health and social care professionals.
