Community Healthcare Nurse
OverviewWe are seeking an experienced Community Healthcare Nurse to join our team in Central London. As a key member of our community healthcare team, you will be responsible for delivering high-quality nursing care to patients in their homes and providing rapid response, rehabilitative, and post-acute care in various community settings.
Job Responsibilities- Deliver skilled, effective, and evidence-based nursing care to patients in a variety of community settings.
- Use enhanced skills and knowledge to assess physical and psychosocial needs and develop comprehensive care plans with patients, carers, and the wider multidisciplinary team.
- Make risk assessments, establishing the safety or otherwise of patients' ability to stay at home or return home safely from another care setting, providing and arranging necessary nursing and supportive packages.
- Registered Nurse (RN) with NMC registration.
- Previous experience of undertaking comprehensive health needs assessment and writing care plans.
- Experience of teaching junior staff/students, peers in-service training settings.
- Ability to demonstrate organisational skills and prioritisation skills.
- Ability to effectively communicate highly complex information verbally and in writing, with a range of people some of whom may have barriers to communication and understanding.
