Job Description
Central London Community Healthcare NHS Trust is seeking an experienced Nurse Practitioner to join our Community Care team in Westminster. As a key member of the team, you will be responsible for supporting patients on the Community Matrons caseload, providing high-quality nursing care and promoting independence.

Your Key Responsibilities
- Providing nursing care and support to patients in their homes and local clinics
- Monitoring vital signs and medication compliance
- Attending outpatient appointments and liaising with other health and social care providers

Person Specification
We are looking for an individual with a strong background in Primary Care, who is able to assess and plan individual care needs, undertake clinical observations, and has venepuncture skills. Experience of working with older frail adults and managing patients with long term conditions would be beneficial. Strong communication and documentation skills, time management skills, and knowledge of using S1 are essential for this role.
