About the Role

We are seeking a dynamic and compassionate Lead Community Healthcare Specialist to join our East North East Community Learning Disability Team. As a key member of our team, you will provide direct clinical care to adults with learning disabilities, working towards improving their lives and preventing admissions to specialist mental health and general health hospitals.

The successful candidate will be based within the East North East CLDT, working Monday to Friday 9am to 5pm. They will provide clinical and operational supervision to junior colleagues, developing intervention toolkits for nurses and holding a caseload including care co-ordination.

Our Learning Disability service has clear clinical governance pathways and supports professional and service development. As a Lead Community Healthcare Specialist, there is an expectation to contribute or lead on service development, service projects, and task groups. Robust supervision structures are in place, with 1:1s with support from the Specialist Nurse Practitioner, peer support through weekly MDT meetings and community nurse meetings.

Key Responsibilities

  • To highlight and lead upon service improvement initiatives
  • To provide support for junior staff and learners
  • To provide evidence-based care
  • To have experience of time management in terms of a caseload or project work
  • To deliver the nursing process in terms of assessment, planning, intervention, and review in a timely manner
  • To experience undertaking various assessments such as biopsychosocial assessments and following clinical pathways
  • To experience risk management
  • To work in a hybrid and flexible way
  • To provide robust managerial and clinical supervision
  • To undertake professional development through accessing opportunities as identified in appraisals
  • To develop training tools and provide training as required
  • To have an understanding of Safeguarding principles

Requirements

  • To be qualified and registered RNLD/RNMH
  • To have experience of working with people with a learning disability
  • To have experience of working with a multi-disciplinary or multi-agency team
  • To be a driver with access to a vehicle
