Senior Practitioner for Independence and Wellbeing

An exciting opportunity has arisen for a Senior Independence and Wellbeing Practitioner to join our team at West Suffolk Hospital. The main purpose of this role is to work with customers to identify and deliver personalised good quality outcomes to the highest standards, supporting and maximising independence and wellbeing.
This involves working within an acute hospital environment to enable customer discharge from hospital, completing Care Act Assessments, assessing functional ability, carrying out Mental Capacity Assessments, promoting digital care technology, and providing advice to customers, their families, and health staff.
- Support community referrals to prevent hospital admissions by linking in with INT coordinators.
- Manage the referral process, including benefits tracking and recording.
- Support communication and awareness raising.
- Make decisions about eligibility, private pathway, and charging.
You will be required to have:
- Good knowledge of Adult Social Care Services.
- Proven ability to explain and describe accurately.
- Excellent negotiation skills to liaise between customers, manager, and service providers.
- Proven experience and understanding of a person-centred, enabling approach to working with people to maximise their independence and wellbeing.
This role offers you the opportunity to join a professional and enthusiastic team, ensuring the provision of valued quality services to vulnerable adults across the West and East of Suffolk. You will work within an acute hospital environment at West Suffolk Hospital, with the possibility of home visits, care home visits, and training opportunities requiring travel to other locations.
