About the Opportunity

Suffolk County Council is seeking a Senior Independence and Wellbeing Practitioner to join our team at West Suffolk Hospital. This is an exciting opportunity to work with customers to identify and deliver personalised outcomes, supporting and maximising independence and wellbeing.

The main purpose of this role is to work within an acute hospital environment, completing assessments, promoting digital care technology, and providing advice to customers, their families, and health staff.

  • Work collaboratively with healthcare professionals to support customer discharge from hospital.
  • Carry out Care Act Assessments and assess functional ability.
  • Make decisions about eligibility, private pathway, and charging.

You will need:

  • Good knowledge of Adult Social Care Services.
  • Proven ability to explain and describe accurately.
  • Excellent negotiation skills to liaise between customers, manager, and service providers.
  • Proven experience and understanding of a person-centred, enabling approach.
