Job Summary

The successful candidate will be a Senior Independence and Wellbeing Practitioner, responsible for working with customers to identify and deliver personalised outcomes, supporting and maximising independence and wellbeing.

This role involves working within an acute hospital environment, completing assessments, promoting digital care technology, and providing advice to customers, their families, and health staff.

  • Provide support to community referrals and manage the referral process.
  • Carry out Mental Capacity Assessments and make decisions about eligibility, private pathway, and charging.
  • Support communication and awareness raising efforts.

You will require:

  • Good knowledge of Adult Social Care Services.
  • Proven ability to explain and describe accurately.
  • Excellent negotiation skills to liaise between customers, manager, and service providers.
  • Proven experience and understanding of a person-centred, enabling approach.
