We are seeking a skilled and compassionate Band 5 Community Staff Nurse to join our team at Livingstone Hospital, Dartford. As a valued member of our Swanley and Rural PCN Team, you will play a vital role in delivering high-quality patient care and contributing to the transformation of health and care services.

With a focus on exceptional patient experiences, you will work across 7 days per week, providing comprehensive nursing care to patients and their families. Your strong communication and organisational skills will enable you to deliver positive outcomes and promote a flexible approach that meets the diverse needs of our community.

Main Responsibilities

As a Band 5 Community Staff Nurse, you will be responsible for:

  • Caring for patients and their families with compassion, empathy, and respect
  • Delivering high-quality nursing care that meets the highest clinical and quality standards
  • Working collaboratively with multidisciplinary teams to ensure seamless care transitions
  • Communicating effectively with patients, families, and healthcare professionals to promote understanding and cooperation
