Senior Clinical Specialist

This exciting opportunity has arisen within our Community Learning Disability Intensive Support Team (IST) in South Staffordshire. As a Band 6 Clinical Nurse Practitioner, you will be responsible for leading a multidisciplinary team to provide high-quality, person-centred interventions to adults with a learning disability.
The IST is part of the Specialist Learning Disability Services and receives internal referrals from CLDT colleagues. You will work collaboratively alongside the Specialist Community Learning Disability Teams, in-patient services, and statutory / independent sector provider services.
As an experienced RNLD or suitably qualified nurse, you will have a strong understanding of current trends and legislation impacting on services provided to people with a learning disability. You will also have experience of providing clinical supervision for others and knowledge of Positive Behaviour Support.
Key Responsibilities- Lead a defined caseload of adults who may be at risk of placement breakdown, including individuals in transition from children's services, specialist inpatient services or moving between community placements.
- Provide person centred assessment and advise care givers and junior staff on implementing and monitoring clinical interventions based on MDT clinical formulation.
- Work closely with your IST MDT colleagues to support adults with complex health care needs, such as Behaviour that Challenges and Mental Health needs.
- An estimated salary of £45,739 - £53,618 per annum.
