TMPW for NHS Health Education England (South West) is seeking an experienced Advanced Care Practitioner to join our LDA Outreach Team.

In this critical role, you will work collaboratively with other healthcare professionals to assess, plan, and deliver complex care packages to vulnerable service users across Devon. You will also provide expert advice and guidance to colleagues from 'The Brook' inpatient unit.

This is a Band 7 role that requires advanced clinical leadership skills, extensive clinical knowledge, and the ability to work autonomously as well as part of a multiprofessional team. The successful candidate will receive a competitive salary of £46,148.00-£52,809.00 per year, plus a relocation package for eligible candidates.
