Outreach Service Coordinator

We are seeking an experienced Learning Disability Specialist Clinician to join our Outreach Team at Devon Partnership NHS Trust. This is an exciting opportunity to work closely with service users, families, and partner agencies to develop and deliver personalized care plans.
About the RoleYour expertise will be essential in assessing, planning, and delivering care packages that cater to the unique needs of each service user. You will also play a key role in fostering strong relationships with partner agencies and families.
Main Responsibilities- Assess, plan, and deliver care packages that prioritize service user well-being and safety.
- Build and maintain positive relationships with partner agencies, families, and service users.
To succeed in this role, you will need:
- Significant experience in learning disability and autism services, with a focus on care package development and delivery.
- Excellent communication and interpersonal skills, with the ability to build strong relationships.
- An advanced understanding of NSF and Mental Health Act/Mental Capacity Act requirements.
Salary: £59,000 - £69,000 per annum.
