Learning Disability and Autism Outreach Practitioner Role

We are seeking a highly motivated and clinically experienced registered clinician to join our newly established Learning Disability and Autism (LDA) Outreach Team at Devon Partnership NHS Trust.

This role involves collaboration with LDA Outreach colleagues, community learning disability and autism teams, and colleagues from "The Brook," a 10-bedded Learning Disability and Autism inpatient unit.

We aim for broad multidisciplinary representation, including Mental Health and Learning Disability nursing, AHPs with Occupational Therapy and Speech and Language Therapy backgrounds.

As a key member of the team, you will be responsible for triaging, assessing, planning, and delivering complex care packages to admission-vulnerable service users across Devon and those discharged from The Brook, ensuring proactive liaison with partner agencies, families, and care providers throughout.

We offer a competitive salary of £46,148.00 and a relocation package as part of this offer.

Main duties of the job

The successful candidate will have effective clinical leadership and practice to the highest evidence-based standards.

You will work closely with the team to develop and implement comprehensive care plans that meet the unique needs of each individual, ensuring that their physical, emotional, and psychological well-being is prioritized.
