Description:

We are seeking an experienced Learning Disability Specialist to join our team in Hertfordshire. As a Community Nurse, you will work closely with individuals with learning disabilities, assessing their healthcare needs and developing tailored care plans to promote their well-being.

Responsibilities:

  • Conduct comprehensive assessments of individuals with learning disabilities
  • Develop and implement care plans to meet individual needs
  • Collaborate with the multidisciplinary team to ensure seamless care delivery

Requirements:

  • Professional registration with Nursing and Midwifery Council (NMC)
  • Strong communication and interpersonal skills
  • Ability to work effectively within a management structure
  • A good understanding of legislation and national guidance relating to learning disability care
