Job Summary
We are recruiting for a Community Health Care Professional to work with individuals who require support with complex physical health needs, challenging behaviour, and complex mental health issues.
As a Registered Learning Disability Practitioner, you will be an integral part of the multidisciplinary team, working together to provide person-centred care and support. The successful candidate will possess excellent networking and communication skills, with the ability to work independently in a community-based environment.
This is a great opportunity to work in a dynamic and supportive team, making a positive impact on the lives of those we serve.
Salary: £38,000 - £50,000 per annum
Requirements:
