Health Care Support Professional

We are seeking a highly motivated and compassionate individual to join our Physical Health Team as a Health Care Support Professional. This is an exciting opportunity to work with individuals who require support with their physical health needs.
Key Responsibilities- Contribute to the assessment of both physical and mental health needs, focusing on improving the physical health of individuals with severe mental illness.
- Work collaboratively with other agencies to deliver a holistic approach to meeting service users' physical health and wellbeing needs.
- Support the implementation of new digital technology for assessing physical health needs.
- Have a good understanding of clinical guidelines for physical health monitoring and identify when action is required due to deteriorating health.
- Acknowledge diversity and promote anti-discriminatory practice/equal opportunities.
- Act calmly in emergencies and respond professionally to stressful and challenging behaviour.
Salary: £26,530 - £29,114 per annum (pro rata)
