Job Title: Clinical Care Specialist

About the Role:

We are seeking a skilled Clinical Care Specialist to join our team at Overbury House, a leading provider of residential, nursing, and dementia care in Norfolk. As a Clinical Care Specialist, you will play a critical role in ensuring the highest standard of care for our residents, working closely with our nursing team to deliver person-centred care.

Key Responsibilities:

* Administer and order medicines as required
* Maintain effective communication with residents, families, and healthcare professionals
* Support new admissions and discharges, ensuring a smooth transition for residents
* Develop and implement individual care plans, including risk assessments and diet plans
* Collaborate with the nursing team to ensure the delivery of high-quality care

Requirements:

* Current NMC registration
* Good knowledge of clinical practices and regulatory frameworks
* Experience in writing detailed care plans and risk assessments
* Ability to work effectively in a team environment
