Job Description

We are seeking an experienced Mental Health Practitioner to join our team at North Halifax Primary Care Network (PCN). As a Mental Health Practitioner, you will play a vital role in improving the lives of our patients by providing high-quality care and support.

Key Responsibilities

  • Provide high-quality patient care, using your expertise and experience to assess, diagnose, and treat patients with mental health conditions.
  • Work collaboratively with other healthcare professionals to develop and implement effective care plans.
  • Communicate effectively with patients, families, and colleagues, using excellent interpersonal and influencing skills.
  • Participate in multidisciplinary team meetings and contribute to the development of care pathways.
  • Develop and maintain strong links with local services and stakeholders to promote collaborative working and consistency of patient care.

Requirements

  • Nursing and Midwifery Registered Mental Health Professional
  • Current UK Professional Registration
  • Clinical Supervision Training and Experience Mentor/Teaching qualification
  • Non-medical Prescriber
  • Independent/Supplementary Nurse Prescribing Qualification
  • Minimum 3 years post-registration experience
  • Evidence of continuing professional development
  • Clinical skills: mental health clinical leadership skills, change management skills, ability to support patients to change lifestyle
  • Excellent communication skills, both written and verbal
  • Ability to communicate difficult messages to patients and families
  • Negotiation and conflict management skills
  • In-depth therapeutic and clinical knowledge and understanding of evidence-based healthcare principles

Salary and Benefits

The salary for this position is between £35,000 - £45,000 per annum, depending on experience and skills. The role offers a full-time contract with 29 days holiday per year plus 8 public holidays and NHS pension. Additional benefits include a company laptop and health care plan after passing probation.
