Job Overview:


We are seeking a highly experienced Mental Health Practitioner to join our Community Frailty Team and Virtual Wards in Calderdale and Huddersfield NHS Foundation Trust.


The successful candidate will work as part of a multi-disciplinary team, providing flexible and individualized care packages to meet the mental health requirements of patients with complex needs.


Main Responsibilities:



  • To provide specialist mental health assessments, planning, implementation, and evaluation of planned treatment for individuals with common mental health problems using evidence-based practice.
  • To develop therapeutic relationships with service users, empowering them to make informed choices about their health and healthcare.
  • To demonstrate clinical leadership through participation in the management of referrals, provision of clinical supervision, and facilitating case review discussions when required.
  • To prescribe medication as an independent nurse prescriber within their own sphere of competence and the bounds of their approved prescribing status.

Requirements:



  • A registered mental health practitioner with relevant professional qualifications and experience.
  • Demonstrated expertise in assessments and treatments of people living with frailty and complex mental health needs.
  • An understanding of current NHS and Mental Health Services, including the Mental Capacity Act.
  • Leadership skills and excellent decision-making capabilities.
  • A working knowledge of National Service Frameworks relating to mental health.
  • The ability to develop systems and processes to improve patient outcomes.

Working Arrangements:


This is a full-time role, offering a competitive salary of £48,644 - £53,887 per annum, pro rata, depending on experience. The post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and a Disclosure and Barring Service check will be required.
