Community Health Support Specialist

We are seeking a highly skilled Community Health Support Specialist - Children's Services to join our team at Great Western Hospitals NHS Foundation Trust. As a key member of the Children's Continuing Care Team, you will provide critical support to children and young people with complex health needs, including those dependent on technology, in their homes or community settings.
Your primary responsibilities will include providing high-quality care, carrying out delegated clinical tasks, and working closely with families and registered nurses to develop and implement individualized care plans. You will also be required to maintain accurate records, participate in ongoing training and development, and adhere to NHS standards and protocols.
A strong understanding of how chronic illness affects families and effective communication skills are essential for this role. You must also be able to work independently, recognize your own limitations, and seek support when needed. In return, we offer a competitive salary of £24,071 to £25,674 per annum, pro rata, and opportunities for growth and professional development.
