Mental Health Crisis Response Practitioner

As a Mental Health Crisis Response Practitioner, you will play a vital role in providing urgent assessment and brief intervention, as well as specific evidence-based therapies to children and young people with mental health problems and significant mental health disorders.
You will work closely with parents, carers, and professionals to understand and support young people through education, advice, and direction on managing difficulties.
Key Responsibilities- Undertake assessments and provide clinical formulation to identify needs and develop appropriate care plans.
- Offer liaison, consultation, and brief intervention to facilitate safe discharge and support care planning.
- Work collaboratively with a multi-disciplinary team to provide urgent assessment and brief intervention.
