We are looking for a skilled mental health practitioner to join our team in North Wales. The ideal candidate will have experience working with children, young people, and families in a healthcare setting and be able to communicate effectively with clients from diverse backgrounds. In this role, you will be responsible for providing crisis response services, conducting assessments, and offering support to individuals in need. You will work closely with other professionals to develop and implement effective care plans that meet the unique needs of each client.

About the Position

This job involves working as part of a multidisciplinary team to deliver high-quality mental health services to children and young people in North Wales. As a community mental health practitioner, you will be based in Wrexham and will be expected to work across the region, visiting clients in their homes or community settings. Your duties will include assessing clients' needs, developing care plans, and delivering therapy sessions. You will also be involved in training and education activities, sharing your expertise with colleagues to enhance the overall quality of service delivery.

Key Responsibilities

The successful candidate will be responsible for:
  1. Conducting assessments to identify clients' needs and develop effective care plans.
  2. Providing therapy sessions, including individual and group work, to support clients' recovery and well-being.
  3. Working collaboratively with other professionals to deliver comprehensive care services.

Requirements and Benefits
