We are currently seeking an experienced Community Dietitian to join our team. As a Community Dietitian, you will be responsible for providing dietetic assessment, advice, and care planning to community patients. You will work alongside other health professionals to provide a multidisciplinary team approach to the care of vulnerable elderly patients with nutritional needs.
**Key Responsibilities:**
- Provide dietetic assessment and advice to community patients.
- Develop and implement care plans to support patients' nutritional needs.
- Work collaboratively with other healthcare professionals to ensure comprehensive patient care.
