About the Role

Healthy Family Support Practitioner – Full Time Vacancy. We are seeking a skilled practitioner to work with children, young people and their families in Harlow.

Key Responsibilities:
  • To work with children, young people and their families on a one-to-one or group basis using various interventions to identify needs and achieve positive outcomes.
  • To work in partnership with parents/carers to empower them and enable them to achieve better outcomes for their children.
  • To work co-operatively with children's services, health agencies and other community-based services to provide an effective service for children, young people and families.
  • Undertake developmental and health reviews of children and young people at key stages of their life.
Requirements:
  • We are looking for a practitioner who is experienced in working with children and/or young people 0-19 years old and their families.
  • Working in an integrated team to support the delivery of the Healthy Child Programme.
  • Promotion of the Baby Friendly Initiative (BFI).
  • Providing evidence-based interventions with families to support their children to become School Ready and meet age-appropriate levels of development.
  • Organising and undertaking Integrated two-year reviews in partnership with the family and the child's Early Years setting.
  • To participate in and deliver ECFWS virtual sessions to ensure all families are reached and have meaningful contact and achievable outcomes.
  • Make contact with families where parental conflict has been identified as a 'risk' factor, providing brief intervention support and advice.
  • Participating and leading on health promotion activities in the family hubs and delivery sites or at external venues and events.
