About the Role

Action for Family Carers is seeking a motivated and experienced individual to lead the delivery of our project supporting Young Carers in our community.

Key Responsibilities

  1. Work with schools, colleges, health and social care providers to help achieve positive outcomes for Young Carers.
  2. Support the positive transition of Young Carers as they become Young Adult Carers.
  3. Identify priorities and direct support to where it can make the greatest impact.

Requirements

  • Passionate about helping disadvantaged young people.
  • Skilled and experienced in working with children and young people.
  • Proactive and organised.
  • Able to manage complex cases including safeguarding issues.
