**Job Description**

We are seeking a highly skilled and empathetic individual to fill the role of Vulnerable Adult Support Specialist.

The successful candidate will be responsible for delivering exceptional care to vulnerable adults across Hertfordshire, ensuring their needs are met through effective support planning and coordination with healthcare professionals.

Responsibilities:

  • Provide responsive and flexible support to Service Users
  • Collaborate with Clinicians and Community Health Professionals to ensure seamless care delivery
  • Conduct thorough assessments of service users' needs and develop tailored support plans
  • Support service users during hospital stays and after discharge, promoting early recovery and preventing readmission
