Job Description

We are seeking an experienced Healthcare and Reablement Specialist to join our team at the NHS. In this role, you will work closely with our clinical team to deliver high-quality care and support to patients with complex needs. Your expertise and compassion will help patients achieve independence and improve their overall quality of life.

You will be responsible for conducting thorough assessments of patients' needs, developing individualized care plans, and providing ongoing support and review. Your strong communication skills and ability to work collaboratively with multidisciplinary teams will enable seamless patient transitions and effective communication.

Main Responsibilities
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Provide high-quality care to patients, adhering to established protocols and guidelines while maintaining flexibility to meet evolving patient needs.
  • Collaborate with multidisciplinary teams to coordinate patient transfers, admissions, and discharges, ensuring smooth transitions and effective communication.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes, contributing to the continuous improvement of our services.
Compensation and Benefits

The annual salary for this position is estimated to be £26,530-£29,114 per annum, depending on experience. As an NHS employee, you will also receive a comprehensive benefits package, including pension schemes, health insurance, and opportunities for professional development.
