About the Role:
This is an exciting opportunity to join our Rapid Response and Intermediate Care team as a Healthcare Assistant Practitioner. You will work closely with our experienced clinicians to deliver high-quality patient care, focusing on rehabilitation and reablement. Key Responsibilities:
  • To perform patient assessments, develop, and implement individualized care plans, promoting independence and well-being.
  • To carry a caseload of non-complex patients, ensuring timely and effective support.

Person Specification:
  • A minimum of 2 years' experience in a patient-facing role, preferably with a reablement or rehabilitation focus.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.

Salary: £26,530 to £29,114 per annum (pro rata)
