We are looking for an ambitious Service Expansion Lead to drive the growth of our innovative Rapid Response & Intermediate Care service in Buckinghamshire.

Your Responsibilities:
  • Develop and implement strategies to expand our Rapid Response & Intermediate Care service, ensuring high-quality patient outcomes.
  • Lead a dynamic team, providing expert interventions and rehabilitation to patients in crisis.
  • Collaborate with healthcare professionals to deliver comprehensive specialist assessments, reviews, and case management services.
  • Design and manage programmes of care to promote health gain and maximise independence for patients.

  • Ability to work effectively in a fast-paced environment, prioritising multiple tasks and deadlines.

Salary: £68,000 - £90,000 per annum
