Job Title: Social Work Specialist

We are seeking an experienced Social Work Specialist to join our Adult Community Mental Health Team at Entrust Social Care Ltd. As a key member of the team, you will be responsible for assessing and managing complex mental health cases, developing and implementing care plans, and collaborating with other professionals to ensure the best possible outcomes for service users.

The successful candidate will have a minimum of 2 years' experience as a Qualified Social Worker in an adult social work setting, with a strong understanding of statutory responsibilities, including the Care Act and safeguarding vulnerable adults and children. They will also possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Conduct comprehensive assessments of service users with complex mental health needs
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each service user
  • Liaise with other professionals, including GPs, hospital staff, and community agencies, to ensure seamless care coordination
  • Monitor and review care plans regularly, making adjustments as needed to ensure optimal outcomes

  • DipSW or equivalent qualification
  • Registration with the HCPC
  • CURRENT DBS
  • Full UK driving license

Salary Range: £38,000 - £42,000 per annum
